understand that the details of my ministry are protected as “confidential”. As such, identifying specific details of my session may not be disclosed to others without my consent except as specified by state law: 1) when not to do so would potentially result in physical harm to myself or others, 2) to report any disclosure/ suspicion of child/elderly neglect or abuse (physical or sexual). Also, I understand that Indigo Heart LLC, HeartSync Ministries, and affiliate partners provide Prayer Ministry and are not licensed counseling services. I acknowledge that each Indigo Heart prayer minister, facilitator, intern or intercessor is solely responsible for the ministry they do. Prayer ministers are trained to varying levels and have varying experience. Some are qualified or certified in HeartSync prayer ministry. I understand that the Indigo Heart team has completed most but not all of the qualification training and so may not know all of the tools of Heartsync Ministry. I understand they reserve the right to say they do not have the skill to go further and redirect me to a divine encounter and then with my permission refer me to another minister or refer me to Heartsync Ministry headquarters . I understand that specific results are not guaranteed and progress may be immediate or take place over extended periods of time. If I have any questions concerning my prayer minister, intercessor or prayer team’s training, experience, or qualifications, I have the right to question them. I understand since Indigo Heart LLC offers prayer ministry and not professional counseling, Indigo Heart LLC does not keep detailed (written) records and will not provide information for lawyers, for disability claims or for other similar matters or legal inquiries. I understand that this ministry may use the services of volunteer prayer facilitators, intercessors, and/or HeartSync Interns. I understand that since it is ministry, the facilitator and intercessor will never offer medical advice. I understand that if I am under the care of or receiving treatment from any medical or mental health care professional I will not modify or terminate any treatment I am receiving, that is prescribed from any such medical or mental health care professional, including any therapy or medication, without first consulting with that medical or mental health care professional. I understand that should I modify or terminate any treatment I am receiving or that is prescribed from any medical or mental health care professional, including any therapy or medication with or without consultation with that medical or mental health care professional, I do so at my own risk and hold Indigo Heart LLC and Heartsync Ministries and those participating in this Heartsync prayer ministry process as harmless for any injury or damage suffered as a result of my decision. I understand that if I am currently taking any medication, or operating under the advice of a professional service, I will allow them (my medical doctor, therapist, counselor, etc.) to confirm any results of prayer ministry received before altering any prescribed course of action. I understand prayer ministers are to consult periodically with their leadership team. Names and specific identifying characteristics will not be routinely discussed or disclosed except where referral to other care/treatment is indicated. I also give my permission for my prayer minister to discuss my prayer needs with the appropriate leadership personnel as necessary. If it is necessary for me to receive additional prayer ministry sessions, I give the leadership team and the ministry team permission to collaborate and evaluate my future ministry needs. I also understand that this ministry has a limited number of prayer facilitators and intercessors who occasionally cancel for extenuating circumstances. In the event that my prayer ministry team chooses to cancel without advanced notice, I agree to either reschedule my appointment or take the next available prayer ministry time. I also understand that I am protected by law from any sexual advances or harassment while undergoing ministry. If at any time I feel offended by anything a member of my prayer ministry team asks, says, or does, I acknowledge that it is my responsibility to tell him or her. If the behavior does not stop immediately, and if the circumstances of the offensive words/actions are such that I am uncomfortable confronting the prayer ministry team member directly, I also acknowledge that it is my responsibility to report it immediately to either the facilitator, certified/qualified HeartSync minister, regional director or Indigo Heart leadership team at: Indigoheart.org. I understand I may be referred to a licensed professional in another ministry, agency or counseling service.

understand that I have voluntarily sought assistance and that I am under no obligation to accept any of the advice, suggestions, proposed actions, prayers or help that I might receive from the ministries, facilitators or intercessors. I understand if at any time either myself, the prayer minister or facilitator may refuse to engage in further communications and/or ministry and be free to terminate sessions with no further ministry, or obligation on anyone’s part. I understand that I will be seeing a prayer minister or facilitator, possibly along with an intercessor and/or HeartSync Intern, who will listen, support, encourage, pray, minister and help me overcome and grow in my Christian life. I understand all ministry is under the direction of the Holy Spirit and that there is no guarantee of healing, recovery or results. Progress may come slowly over the course of time and through multiple sessions.

understand and will defend, indemnify and hold harmless Indigo Heart LLC and HeartSync Ministries and affiliate partners from any and all claims, lawsuits, demands, causes of action, liability, loss, damage and/or injury of any kind whatsoever. This includes without limitation all claims for emotional distress, monetary loss, property damage, equitable relief, personal injury and/or wrongful death, whether brought on by the individual or other entity, or imposed on by a court of law or federal, state or local governmental body or agency arising out of, in any way whatsoever, any acts, omissions, negligence or willful misconduct on the part of the participant, leaders, teachers, ministers, officers, owners, personnel, employees, agents, contractors, invitees, or volunteers. This indemnification applies to and includes, without limitations, the payment of all penalties, fines, judgements, awards, decrees, attorneys’ fees and related costs or expenses, and all reimbursements to Indigo Heart LLC and HeartSync Ministries for all legal fees, expenses and costs incurred by it. No supplement, modification or amendment of this agreement shall be binding unless signed by both parties. No waiver of any default shall constitute a waiver of any other default or breach, whether of the same or other covenant or condition. No waiver, benefit, privilege or service voluntarily given or performed by a party shall give the other party any contractual right by custom, estoppel or otherwise.
